If you're scheduled for a LASIK evaluation, please print and complete "Patient Information" & "Medical History" prior to your appointment.
LASIK evaluation tests require a patient to focus on distance vision, therefore we suggest you avoid near vision focusing immediately prior to testing.
In addition, the document "What to Expect" may offer some helpful information regarding your upcoming LASIK evaluation.
